Tara Delene Hook (Smith) passed away in Moraine, Ohio unexpectedly after a brief illness on June 29, 2026. She was born on August 15, 1970, in Kettering, and her life was marked by deep devotion to family and a profound sense of care for those around her.
A devoted Christian, Tara’s faith anchored her through life’s joys and sorrows. She lived her beliefs every day, faithful and spiritual in both word and deed.
Tara graduated from West Carrollton High School with the Class of 1988. Eager to serve others, she pursued further education at Fortis College, where she earned her Licensed Practical Nurse (LPN) license. Tara’s commitment to healthcare was evident throughout her career. She recently began a new position as a patient scheduler for home health care services and was employed at Buckeye Home Health Care, finding a work family that valued and appreciated her, giving her the respect and gratitude she was due.
Described by all who knew her as loving and giving, Tara approached every relationship and responsibility with selflessness and kindness. Her nurturing disposition and generosity touched countless lives, both at work and in her personal relationships. A faithful and spiritual woman, Tara was a devoted Christian, whose compassion and gentle spirit were intrinsic to her character.
Tara’s life was enriched by the time she spent with family and friends. She found joy in comedy, listening to music, vacationing on the beach, and taking care of animals—acts that reflected her nurturing nature and zest for life. Those closest to her cherished her unwavering support and found in her a source of comfort, laughter, and enduring love.
Tara leaves behind her loving husband, Shawn, who was her steadfast partner through life’s journey. She is also survived by her sisters, Casey (Rick) Reed and Carol Smith, and her nephews Adam, Kyle, and Ryan Reed. Each of them carries a part of Tara’s spirit—her generosity, her devotion, and her unwavering support.
She is now reunited with beloved family members who preceded her in death: her grandparents Herbert and Margaret Loy, and Blair Smith and Jean Smith-Compton; as well as her parents, Gary Smith and Karen Smith. Their memories and the lessons of love Tara learned from them continued to shape her throughout her life.
The memory of Tara will be honored and cherished by all who knew her. Her selfless compassion, professional dedication, and unwavering faith and love have left a lasting imprint on the hearts of many, and she will be remembered with deep respect and affection.
