Chuck "Charlie" Smith, age 38 of Crystal Lake, passed away Saturday, October 11, 2014 in San Antonio, TX of a sudden heart attack. He was born May 12, 1976 in Xenia, OH to Chuck and Mavis (Yeary) Smith.
He is survived by his wife, Christy Smith; children, Charles Robert, Caitlynn and Christopher; his parents; brother, Andy Smith and children, Brennan, Mason and Abbigail; mother and father-in-law, Sandy and Bob McKelvy; sister-in-law, Pam and numerous extended family and friends.
In lieu of flowers, donations may be made to Chuck Smith Memorial Fund at any Security National Bank.
